My Care My Home have exciting opportunities for experienced Support Workers, to join our new Hospital Discharge service.

My Care My Home have been providing care and support to individuals within their own homes in England and Wales since 2016. 

For this role you will be working on double-up calls providing care and support to individuals upon their discharge from hospital throughout Monmouthshire; ensuring they are receiving the correct care to meet their needs through a trusted assessor model of support.

About the role:

You will be ‘buddying up’ with another staff member and working as part of a larger team on alternate shifts.

Working patterns will be between the times of 7.00am to 3.00pm and 3.00pm to 10.00pm. You must be prepared to work every other weekend or 1 day each weekend and to have your rota changed at short notice when on shift to ensure that patients can be discharged in a timely manner.

Travel within the service catchment area is essential for this role along with a full driving licence and access to your own vehicle with 'business use' car insurance.

Depending on your availability, we can offer part-time and full-time contracts with the option of extra bank hours.

 

Duties include:

• Prompting/support with personal care and dressing (which could include assistance with intimate personal care).
• Assisting with medication –prompting or administering medication.
• Meal preparation.
• Hoisting and supporting transfers
